Poxviridae is a family of viruses. Humans, vertebrates, and arthropods serve as natural hosts. There are currently 69 species in this family, divided among 28 genera, which are divided into two subfamilies. Diseases associated with this family include smallpox. Four genera of poxviruses may infect humans: orthopoxvirus, parapoxvirus, yatapoxvirus, molluscipoxvirus. Orthopox: smallpox virus (variola), vaccinia virus, cowpox virus, monkeypox virus; Parapox: orf virus, pseudocowpox, bovine papular stomatitis virus; Yatapox: tanapox virus, yaba monkey tumor virus; Molluscipox: molluscum contagiosum virus (MCV).The most common are vaccinia (seen on Indian subcontinent) and molluscum contagiosum, but monkeypox infections are rising (seen in west and central African rainforest countries). This video is on poxviruses .Poxviruses are brick-shaped (240 nm by 300 nm) and have a complex internal structure including a double-stranded DNA genome.Replication of the poxvirus involves several stages and take place in cytoplasm.Infection is usually caused by invasion through broken skin and in most cases remains localized. Human monkeypox is acquired by contact or by airborne transmission to the respiratory mucosa. Initial viremia during the incubation period spreads infection to internal organs; a second viremia then spreads the virus to the skin.The diagnosis is often suggested by the presence of skin lesions and a history of contact with human or animal cases. Diagnosis is confirmed by electron microscopy and/or virus isolation.

Newer vaccines have been developed of which one has been approved for prevention of monkeypox Monkeypox is caused by monkeypox virus, a member of the Orthopoxvirus genus in the family Poxviridae. Monkeypox is usually a self-limited disease with the symptoms lasting from 2 to 4 weeks. Severe cases can occur. In recent times, the case fatality ratio has been around 3–6%. Monkeypox is transmitted to humans through close contact with an infected person or animal, or with material contaminated with the virus. Monkeypox virus is transmitted from one person to another by close contact with lesions, body fluids, respiratory droplets and contaminated materials such as bedding. Monkeypox is a viral zoonotic disease that occurs primarily in tropical rainforest areas of central and west Africa and is occasionally exported to other regions. An antiviral agent developed for the treatment of smallpox has also been licensed for the treatment of monkeypox. The clinical presentation of monkeypox resembles that of smallpox, a related orthopoxvirus infection which was declared eradicated worldwide in 1980. Monkeypox is less contagious than smallpox and causes less severe illness. An infectious disease among in humans and other animals caused by the Monkeypox virus, that belongs to the genus Orthopoxvirus in the family Poxviridae. (Prodromal symptoms prior to the emergence of the rash) Incubation period is 7-14 days. • headache • fever • fatigue • muscle pain • lymphadenopathy: Swelling of lymph nodes near the ear and jaw, in the neck, or in the groin. These prodromal symptoms are similar to chickenpox, measles, and smallpox, except lymphadenopathy. (Symptoms) The duration is 2-4 weeks. • rash that forms blisters and crusts over Within a few days of the fever, they appear first on the face and then palms, soles, mouth, genitals, and eyes in a centrifugal distribution. They begin as small flat spots and then becoming small bumps filled with at first clear fluid and then yellow fluid, which subsequently burst and scab over. Cases may be severe, especially in children, pregnant women, or people with suppressed immune systems. (Transmission) • from animal to human: By an animal bite or direct contact with an infected animal's bodily fluids. Using natural reservoir as food. • from human to human: By respiratory (airborne) contact or contact with an infected person's bodily fluids. The virus is found mainly in tropical rainforest regions of Central and West Africa. African rodents (e.g. Gambian pouched rats, dormice, African squirrels) and non-human primates may harbor the virus as the natural reservoir and infect people. The cases occurred outside of Africa linked to international travel or imported animals. (Comparison to smallpox) Variola virus which causes smallpox also belongs to the genus Orthopoxvirus. Monkeypox is similar to smallpox. • less transmissible • milder rash • lower mortality rate (Diagnosis) • polymerase chain reaction (PCR) testing of samples from skin lesions: Usually inconclusive because the virus does not remain very long in the blood. • lymphadenopathy during the prodromal stage: Differential diagnosis to distinguish from other rash inducing diseases. (Prevention) • smallpox vaccine: Monkeypox virus (that causes monkeypox) and Variola virus (that causes smallpox) are in the genus Orthopoxvirus. (Treatment) • tecovirimat: For several poxviruses (Poxviridae), that includes Monkeypox virus. • brincidofovir: Smallpox treatment. • supportive care: E.g. antipyretic, fluid balance, and oxygenation. In case that secondary infection is suspected: • empirical antibiotic therapy: For bacterial infection. • aciclovir: For varicella zoster infection.

Monkeypox explained. Monkeypoxvirus (MPXV) is a double-stranded DNA virus, a member of the orthopoxvirus genus within the Poxviridae family. Poxviruses cause disease in humans and many other animals; infection typically results in the formation of lesions, skin nodules or disseminated rash. Other orthopoxvirus (OPXV) species pathogenic to humans include cowpox virus, and variola virus (causing smallpox, which has been eradicated). Vaccinia virus is also an OPXV that has been used to vaccinate people and was a key tool for the eradication of smallpox, achieved in 1980. MPXV is named due to its initial detection in monkeys. MPXV can primarily be found in rodents, however the reservoir is undetermined. There are two known clades of MPXV, one endemic in Western Africa and one in the Congo Basin region.
